Enrollment & Co-Oping Paperwork
Before your child(ren) can begin classes at CPNS, there’s some essential paperwork to complete, especially if your family is co-oping. But don’t worry — We’ve got you covered! This ⭐️ interactive guide ⭐️ will walk you through the entire process.
Use this page as a checklist and track the progress of your paperwork submission. Mark each action item as complete if 1) you’ve successfully submitted a form / completed a step, OR 2) if the action step does not apply to you.

Please be aware that you will likely not be able to complete all required paperwork in one sitting. Depending on your child’s student status, or your co-oper status, you may need to make a doctor’s appointment, get a background check, take a training course, or wait a few days/weeks for your doctor to return medical paperwork.
Aside from the Co-Oper Release of Information form (Due June 30th, 2024), the deadline for submitting all other Enrollment and Co-Oping paperwork is August 15th, 2024. So give yourself ample time to complete your paperwork!

Please submit all forms directly on your Jovial family portal unless otherwise stated.

You may not be able to find it if you are trying to access the "2024-25 New Co-Oper Background Check" form before August. New co-opers will not be given access to the form or instructions August 1st, 2024, as we are not allowed to accept background check results too early.

No - In order to remain in compliance with state laws, and for the health and safety of your child(ren), they may only begin attending classes at CPNS once all student enrollment paperwork has been turned in.

No - In order to remain in compliance with state and federal laws, and for the health and safety of our children, caregivers may NOT co-op until they've turned in all required co-oping paperwork.
Regardless of when co-oping paperwork is turned in, each family's co-oping responsibilities remain the same. We may have to nullify a caregiver's co-oping role if co-oping paperwork is significantly delayed, in which case, CPNS will discuss remaining options with the affected co-oper.
