APPLICABLE TO: NEW CO-OPERS ONLY
Each new co-oper must schedule a *medical physical and have an authorized medical provider complete the Medical Evaluation for Child Care Form to confirm they are physically capable to work with young children.
In addition, CPNS follows state and CDC health and safety guidelines and encourages all co-opers to stay updated with recommended vaccinations, including the COVID-19 vaccine.
*Note: If you had a physical in the last 6 months, your provider may complete this form without a new exam. A TB test is only needed if the provider deems it necessary.
Before submitting the “Medical Evaluation for Child Care” form, please double check for these common errors:
Make sure you signed and dated the “Authorization for Release of Medical Information” box.
Confirm that the provider checked “Yes” for the following boxes:
“Risks and Symptoms screening competed (required)”
“2-6 years of age” under question #5
Ensure the provider signed and dated the form and included their full name and credentials, or stamped the form with their credentials.

There are several options. You could:
1. Make an appointment for a medical physical with your own primary care physician (PCP).
2. If you’ve had your annual physical completed within the last 6 months, ask if your PCP will complete the form without an appointment.
3. Get an “employment physical” or “child care physical” at a pharmacy clinic (e.g., CVS, Walgreens) or an urgent care (e.g., MedStar, Patient First). Always call ahead to ensure availability.
