If none of the above apply to your child, you do NOT need to complete these forms and can proceed to the next action item by clicking “Mark As Complete / Skip Step” below.
If your child is receiving any medications or specialized health care services, you will need to complete the appropriate Medication Authorization and/or Individualized Treatment/Care Plan form.
Please download and submit the form relevant to your child’s health needs/conditions:
(General) Medication Administration Authorization Form (MSDE OCC Form 1216)
Required if your child requires the administration of a prescription OR non-prescription medication during school hours (requires physician’s signature).
Asthma Medication Administration Authorization Form (MSDE OCC Form 1216A)
Required if your child requires the administration of an inhaler or nebulizer during school hours (requires physician’s signature).
Allergy & Anaphylaxis Medication Administration Authorization Form (MSDE OCC Form 1216B)
Required if your child has a diagnosed allergy that requires the administration of Epinephrine medication and/or antihistamine during school hours (requires physician’s signature).
Required if your child requires seizure medication during school hours (requires physician’s signature).
Special Health Condition Individualized Care Plan Checklist (MSDE OCC Form 1216D)
Required if your child has special health care needs and/or individualized treatment care plans, procedures, or medications. Attach this form to a written treatment/care plan.
Tip: If you choose to upload your forms via the Jovial portal (preferred), you will need to scan and compile the relevant medical forms into ONE PDF FILE (you will not be able to upload separate PDF documents). Documents in the form of image files can be uploaded separately, but please ensure images are clear enough to print.
