APPLICABLE TO: ALL CPNS STUDENTS

This online questionnaire helps us get to know your child and family better. A new form must be submitted for each child annually on the Jovial platform.

All information is kept confidential and is solely used to better meet your child’s and family’s needs. We appreciate as many details as you’re comfortable providing. You may leave any questions you prefer not to answer blank.

1. Log into your Jovial Family Portal
2. Click on “Forms”
3. Click on “2024-25 Know Our Child and Family”
4. Fill out the online form and click “submit” when you are finished.

Tip: Jovial forms time out after one hour. If you need more time, type your answers in a separate document and copy-paste them into the online form later.
